The Transitional Housing Placement Program (THPP) is a community care licensed placement option for foster and probation youth in out-of-home placement. The goal of THPP is to help participants emancipate successfully from foster care by providing a safe living environment for them to practice their independent living skills. Program participants may live alone (with the approval of their social worker) or with roommates in apartments and single-family dwellings where regular support and supervision is provided by THPP staff, county social workers, and the Independent Living Program coordinator. Services ProvidedTHPP participants receive regular visits from a case manager, educational guidance, employment counseling, and assistance in reaching the emancipation goals outlined in participants' transitional independent living plans. Transitional Housing for Emancipated Foster YouthThe Transitional Housing Placement Plus (THP-Plus) program provides affordable housing and comprehensive supportive services for up to 24 months to help former foster care and probation youth ages 18 to 24 make a successful transition from out-of-home placements to independent living. Housing and support services are funded by the Human Services Agency and provided through our community partners: Larkin Street Youth Services, First Place for Youth, Salvation Army and Edgewood Center for Children and Families. Services ProvidedSupport services include assistance finding housing, a rent subsidy, case management, employment related services, educational advocacy, mentoring, post program housing assistance, and alumni services. Participation in the THP-Plus is subject to the individual's county of origin's participation in the statewide THP Plus program and the availability of safe and affordable housing. The maximum time for THP-Plus participation is 24 cumulative months.
