Are you a senior or an adult with a disability who is living in Laguna Honda Hospital or another care facility? Are you still living at home, but at risk of facility placement because of safety issues? Or are you the advocate for such a person? If so, the Community Living Fund may be able to help you.

Community Living Fund case managers help clients evaluate all the possible funding sources and service options available to them. The Community Living Fund works to fill in the gaps so persons can live safely at home.

Who Is Eligible?

Any San Francisco adult with a functional impairment or medical condition that requires care and who needs assistance to either avoid moving to an institution or to leave one, and whose annual income is up to 300% of the federal poverty level (currently $31,200).  CLF Referral Form

What Support Is Provided?

Case managers help clients obtain services from a variety of sources. When Community Living Funding is appropriate, it may support items ranging from the purchase of equipment to modifications to the residence or payment for needed support services. Because every individual is unique, there is no set list of items that may be purchased with Community Living Funds.
