The San Francisco Partnership for Community-Based Care and Support was implemented by the Department of Aging and Adult Services to strengthen and sustain neighborhood partnerships in creating a well-coordinated system of long-term care services delivery for seniors and adults with disabilities.  The goals of the partnership are to improve coordination of home and community-based services, advance quality of life and quality of care, and promote home and community-based service options.

To reflect the city's diverse racial, ethnic, and cultural backgrounds, the partnership is represented by the African American, Latino, Asian & Pacific Islander, and LGBT communities.  For more information on the individual community partnerships, please click on the links below.
