San Francisco has adopted a housing first model to end chronic homelessness. These programs emphasize placement of individuals in permanent supportive housing, where they have access to on-site case managers and a menu of supportive services that stabilizes individuals and keeps them housed. This model has proven to be the most successful approach to ending homelessness. HSA’s Supportive Housing programs include Housing First and Shelter + Care.
