A special meeting of the Human Services Commission was held on Thursday, June 17, 2004, in the Ronald H. Born Auditorium at 170 Otis Street, San Francisco.

President Gaines called the meeting to order at 9:40 a.m. noting that the Commission was present.

FAREWELL |
Vice President Yamasaki announced with regret that President Gaines had submitted his resignation to the Mayor effective June 21, 2004. President Gaines has accepted a career opportunity that will take him to Washington, D.C. As a token of the esteem and affection in which President Gaines is held, Executive Director Rhorer joined Vice President Yamasaki and Commissioners Martinez and Stewart, and Deputy Directors Buick and Kipper, in presenting President Gaines with a commemorative gavel honoring him for his compassionate and distinguished service to the Commission and the Department over these past nearly seven years.

On motion by Vice President Yamasaki, seconded and carried, the Commission adopted the agenda with the following changes:
Item III.B. Jerry Lew & Associates; Item III.L Mission Neighborhood Health Center; and Item III.S. Kohler, Wargo, Coleman, Moore --- continued to the June 24, 2004 Regular Meeting.

William Beiersdorfer, CalWORKs Program Support Analyst, presented the request to renew the contract with Elizabeth Loyola Swason, M.A.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to renew the contract with ELIZABETH LOYOLA SWASON, M.A. to provide Academic Services to Cal-Learn, TAPP, and START Clients; during the period of July 1, 2004 through June 30, 2006; in a total contract amount not to exceed $93,600

GOLDEN BEAR |
John Tsutakawa, Family and Children’s Services Contract Liaison, presented the request to renew the contract with Golden Bear.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to renew the contract with GOLDEN BEAR for provision of Children’s System of Care Coordinator; during the period of July 1, 2004 through June 30, 2005; in a total contract amount not to exceed $90,000.

ARLENE HYLTON |
John Tsutakawa, Family and Children’s Services Contract Liaison, presented the request to renew the contract with Arlene Hylton.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to renew the contract with ARLENE HYLTON for provision of Independent Living Skills Service Coordination; during the period of July 1, 2004 through June 30, 2005; in a total contract amount not to exceed $112,000.

JAPANESE COMMUNITY YOUTH COUNCIL |
John Tsutakawa, Family and Children’s Services Contract Liaison, presented the request to renew the contract with Japanese Community Youth Council.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to renew the contract with JAPANESE COMMUNITY YOUTH COUNCIL for provision of Early Independent Living Skills Services; during the period of July 1, 2004 through June 30, 2007; in a total contract amount not to exceed $538,086.

JAPANESE COMMUNITY YOUTH COUNCIL |
John Tsutakawa, Family and Children’s Services Contract Liaison, presented the request to modify the contract with Japanese Community Youth Council.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to modify the contract with JAPANESE COMMUNITY YOUTH COUNCIL for provision of Transitional and Aftercare Independent Living Skills Services; during the period of July 1, 2004 through June 30, 2006; in the additional amount of $28,011 for a revised total contract amount not to exceed $322,247.

CITY COLLEGE OF SAN FRANCISCO |
John Tsutakawa, Family and Children’s Services Contract Liaison, presented the request to modify the contract with City College of San Francisco.
On motion by Vice President Yamasaki, seconded and carried, the Commission excused Commissioner Martinez from the vote on Agenda Item III.G. City College of San Francisco and Agenda Item III.Q. San Francisco Community College District in order to avoid a conflict of interest.
On motion by Commissioner Stewart, seconded and carried with Commissioner Martinez excused from the vote, the Commission granted authorization to modify the contract CITY COLLEGE OF SAN FRANCISCO for provision of Community Based Organization Training; during the period of July 1, 2004 through June 30, 2005; in the additional amount of $380,000 for a revised total contract amount not to exceed $899,000.

GLIDE MEMORIAL UNITED METHODIST CHURCH |
Laura Gerard, Housing and Homeless Program Assistant, presented the modify the contract with Glide Memorial United Methodist Church.
On motion by Vice President Yamasaki, seconded and carried, the Commission granted authorization to modify the contact with GLIDE MEMORIAL UNITED METHODIST CHURCH to provide delivered meals to two homeless shelters; during the period of July 1, 2004 through June 30, 2005; for the additional amount of $58,566 for a revised total contract amount not to exceed $1,737,033.

TENDERLOIN HOUSING CLINIC |
Scott Walton, Supportive Housing Program Manager, presented the request to renew the contract with Tenderloin Housing Clinic.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to renew the contract with TENDERLOIN HOUSING CLINIC for provision of the Single Room Occupancy Housing Program; during the period of July 1, 2004 through June 30, 2005; in a total contract amount not to exceed $6,135,419.

Scott Walton, Supportive Housing Program Manager, presented the request to modify the contract with Tenderloin Housing Clinic.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to modify the contract withTENDERLOIN HOUSING CLINIC for extend the Modified Payment Program; during the period of July 1, 2004 through September 30, 2004; for the additional amount of $363,133 for a revised total contract amount not to exceed $3,742,827.

HOMELESS PRENATAL PROGRAM |
Cindy Ward, Housing and Homeless Family Programs Manager, presented the request to enter into a new contract with Homeless Prenatal Program.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to enter into a new contract with HOMELESS PRENATAL PROGRAM for provision of Housing Assistance Services; during the period of July 1, 2004 through June 30, 2006; in a total contract amount not to exceed $860,808.

COMMUNITY AWARENESS AND TREATMENT SERVICES |
Joyce Crum, Deputy Director Homeless and Housing, presented the request to renew the contract with Community Awareness and Treatment Services.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to renew the contract with COMMUNITY AWARENESS AND TREATMENT SERVICES for provision of South Beach Resource Center; during the period of July 1, 2004 through June 30, 2005; in a total contract amount not to exceed $673,758.

NATIONAL COUNCIL ON CRIME AND DELINQUENCY |
Daniel Kelly, Strategic Planning Coordinator, presented the request to enter into a new contract with National Council on Crime and Delinquency.
On motion by Commissioner Martinez, seconded and carried, the Commission granted authorization to enter into a new contract with NATIONAL COUNCIL ON CRIME AND DELINQUENCY for provision of Safe Measures Database Package; during the period of July 1, 2004 through June 30, 2007; in a total contract amount not to exceed $67,500.

EMERGENCY HOUSING AND ASSISTANCE GRANT |
David Curto, Director – Office of Contract Compliance, presented the request to fully expend the Emergency Housing and Assistance Grant.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to fully expend the EMERGENCY HOUSING AND ASSISTANCE GRANT; for the renovation of 150 Otis Street – first floor for a temporary winter shelter; in a total grant amount not to exceed $467,500.

CEE ARCHITECTS |
David Curto, Director – Office of Contract Compliance, presented the request to enter into a new contract with Cee Archtects.
On motion by Vice President Yamasaki, seconded and carried, the Commission granted authorization to enter into a new contract with
CEE ARCHITECTS for provision of Architectural and Project Management Services for the renovation of 150 Otis Street; during the period of June 18,2004 through December 31, 2004; in a total contract amount not to exceed $ 74,653.

SAN FRANCISCO COMMUNITY COLLEGE DISTRICT |
James Whelly, Employment Services Section Manager, presented the request to renew the contract with San Francisco Community College District.
On motion by Vice President Yamasaki, seconded and carried with Commissioner Martinez excused from the vote, the Commission granted authorization to renew the contract with SAN FRANCISCO COMMUNITY COLLEGE DISTRICT to provide Work Study; during the period of July 1, 2004 through June 30, 2005; in a total contract amount not to exceed $115,000.

WRITE TOUCH COMMUNICATION
|
James Whelly, Employment Services Section Manager, presented the request to renew the contract with Write Touch Communications.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to renew the contract with WRITE TOUCH COMMUNICATIONS to provide resume writing services to Department of Human Services Clients; during the period of July 1, 2004 though June 30, 2006; in a total contract amount not to exceed $40,000.
