The regular monthly meeting of the Human Services Commission was held on Thursday, November 17, 2005 in the Ronald H. Born Auditorium at 170 Otis Street San Francisco.

On motion by Vice President Martinez, seconded and carried, the Commission approved the minutes of the October 27, 2005 Regular Meeting as circulated.

EMPLOYEES OF THE MONTH |
President Yamasaki announced the selection of Senior Personnel Officer Patricia Burns as the November 2005 Employee of the Month. The award honors Pat as an outstanding leader and exemplary employee. She was given an engraved photo album and her name has been engraved on the Employee of the Month Plaque.

CONSENT CALENDAR
|
On motion by Commissioner Murphy, seconded and carried, the Commission ratified actions taken by the Executive Director since the October 27, 2005 regular meeting in accordance with Commission authorization of June 23, 2005:
A. Submission of requests to encumber funds in the amount of $713,407 for purchase of services or supplies;
B. Submission of temporary requisitions DP91952 through DP91614 for possible use in order to fill positions on a temporary basis;
C. Submission of report of 12 Temporary appointments made during the period of 10.13.05 through 11.9.05

HOMELESS PRENATAL PROGRAM |
Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Homeless Prenatal Program contract.
On motion by Vice President Martinez, seconded and carried, the Commission granted authorization to modify the Homeless Prenatal Program contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $61,381 for a revised total contract amount not to exceed $421,381.

ASIAN PRENATAL ADVOCATES |
Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Asian Pernatal Advocates contract.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to modify the Asian Perinatal Advocates contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $61,381 for a revised total contract amount not to exceed $815,753.

Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Urban Services YMCA contract.
On motion by Commissioner Dearman, seconded and carried, the Commission granted authorization to modify the Urban Services YMCA contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $61,381 for a revised total contract amount not to exceed $377,829.

Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Mt. St. Joseph/St. Elizabeth contract.
On motion by Vice President Martinez, seconded and carried, the Commission granted authorization to modify the Mt. St. Joseph/St. Elizabeth contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006 for an additional $61,381 for a revised total contract amount not to exceed $782,360.

INTER CITY FAMILY SUPPORT AND RESOURCE NETWORK, INC. |
Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Inter City Family Support and Resource Network, Inc. contract.
On motion by Commissioner Dearman, seconded and carried, the Commission granted authorization to modify the Inter City Family Support and Resource Network, Inc. contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $138,581 for a revised total contract amount not to exceed $945,108.

RILEY CENTER |
Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the St. Vincent De Paul/Riley Center contract.
On motion by Vice President Martinez, seconded and carried, the Commission granted authorization to modify the St. Vincent De Paul/Riley Center contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $61,381 for a revised total contract amount not to exceed $290,433.

INSTITUTO FAMILIAR
DE LA RAZA |
Family and Children’s Services Contract Liaison John Tsutakawa presented the request to modify the Instituto Familiar De La Raza contract.
On motion by Commissioner Stewart, seconded and carried, the Commission granted authorization to modify the Instituto De La Raza contract for enhanced visitation on site; during the period of November 1, 2005 through June 30, 2006; for an additional $188,727 for a total contract amount not to exceed $829,930.

HAMILTON FAMILY CENTER |
Housing and Homeless Family Programs Manager Cindy Ward presented the request to renew the Hamilton Family Center contract.
On motion by Commissioner Dearman, seconded and carried, the Commission granted authorization to renew the grant with
HAMILTON FAMILY CENTER for the operation of transitional housing and the provision of supportive services; during the period of January 1, 2006 through December 31, 2007; in a total grant amount not to exceed $1,504,458.

PROVIDENCE FOUNDATION |
Housing and Homeless Family Programs Manager Cindy Ward presented the request to modify the contract with Providence Foundation.
On motion by Vice President Martinez, seconded and carried, the Commission granted authorization to modify the contract with
PROVIDENCE FOUNDATION for provision of the Winter Shelter Program; during the period of December 1, 2005 through March 31, 2006; for an additional $251,228 for a revised total contract amount not to exceed $640,009.

PUBLIC SECTOR CONSULTANTS |
Office of Contract Compliance Director David Curto presented the request to modify the contract with Public Sector Consultants.
On motion by Commissioner Murphy, seconded and carried, the Commission granted authorization to modify the contract with
PUBLIC SECTOR CONSULTANTS for CalWIN support services; during the period of August 1, 2005 through June 30, 2006; for an additional $75,000 for a revised total contract amount not to exceed $386,600.
