What is the Partnership?
The San Francisco Partnership for Community-Based Care & Support is a network of more than 70 private and public organizations, working to improve community-based services for older adults and adults with disabilities in San Francisco. 

Since its formation in May 2004, the Partnership has become an extensive network of service providers dedicated to strengthening the system of community-based care and support.  This Partnership is becoming the tangible, visible association of home and community-based long-term care and supportive services for older adults and adults with disabilities in San Francisco.

The purpose of the Partnership is threefold: (1) to enhance collaboration, cooperation, and communication between member agencies in the delivery of services; (2) to promote the services provided by member agencies via a multi-faceted media campaign; and (3) be an increasingly visible and powerful voice in service organization and coordination.

What benefits do Partnership members receive?

  • Members become identified with an extensive network of community-based long term care and supportive service providers.
  • Communication with the members of this network, which includes some of the most innovative service providers in the nation. This is building an increasingly powerful voice in community-based service organization and service coordination as part of the network.
  • Participate in the promotion of services provided through agencies in the Partnership via a multi-faceted media campaign using citywide and community newspapers, bus shelters, billboards, mailers, television, radio.
  • Be continually alerted to expanded resources and opportunities for marketing Partnership services, and for pro-active public relations efforts.

How do Partnership members participate?

  • Provide culturally sensitive and accurate information about their programs and services.
  • Offer reception services and telephone messages that are welcoming and informative for consumers and caregivers.
  • Assist consumers and caregivers to get the help they need at their agencies - or by making appropriate referrals to other agencies within the Partnership.
  • Learn about other community-based services that might benefit clients.
  • Work with other service providers to eliminate service gaps and duplications.
  • Promote positive images of aging and disability.
  • Attend two Partnership luncheon meetings each year.

Participate, if possible, in at least one Partnership committee or implementation workgroup.

Partnership Workgroups

1. Case Management Collaboration Workgroup
This workgroup consists of a citywide network of public and non-profit case management providers working together, through the development a case management collaborative, to improve coordination and services for vulnerable adults. This group includes representatives from DHS, Curry Senior Center, Episcopal Community Center, SFGH, St. Luke's Hospital, LHH, No. Cal. Presbyterian Homes and Services (NCPHS), Network for Elders, Family Service Agency, Self Help for the Elderly.  (Chair: Ramona Davies, NCPHS)


2. Homecare Workforce Recruitment and Retention Workgroup
This workgroup is now implementing objectives for improving the recruitment, training, and retention of homecare workers.  They are preparing a concept paper to develop a homecare worker training institute for all homecare workers in San Francisco. This group consists of representatives from public and private homecare agencies citywide, including consumers, advocates, and homecare workers, PECC, and UCSF. (Chair: Margy Baran, IHSS Consortium)


3. Neighborhood Partnerships Implementation Workgroup
This workgroup is facilitating activities to strengthen culturally appropriate services in underserved communities, focusing on four community partnerships: (1) African American, (2) Asian/Pacific Islander (API), (3) Latino, and (4) Lesbian, gay, bisexual, and transgender (LGBT) communities. Each of the four communities meets separately to determine priorities, then reports back to and receives guidance and support from the larger workgroup. Among other projects, teaching and learning sessions are hosted by each of the individual partnership teams.  (Chair: Rotating chairs within each community partnership)


4. Public Relations and Marketing Implementation Workgroup
This workgroup, along with Wide Angle Communications (a public relations firm), is implementing a comprehensive and multi-faceted public relations and media campaign to: (1) promote the idea that long term care does not only mean nursing home care, and (2) promote positive images of older adults and adults with disabilities. This workgroup includes marketing and development staff from community-based organizations, as well as members of the Aging and Adult Services Advisory Council media relations committee.  This group includes ILRC, On Lok, DHS, Rose Resnick Lighthouse for the Blind, Institute on Aging, Community Clinic Consortium, DAAS Advisory Council on Aging, (Chair: Eleanor Jacobs, San Francisco Community Clinic Consortium)
