Join us to prepare for making decisions about your medical care at the end of life or during a serious illness. Advance Care Planning ensures your decisions will be honored as well as a less stressful experience for you and your loved ones.

Advance Care Planning workshop

This workshop covers how to plan your medical care, including how to select someone to represent you and how to effectively communicate your treatment wishes with doctors, family, and friends.
