Home-Delivered Meals Program
The Home-Delivered Meals Program delivers meals to homebound adults who are unable to shop for or prepare their own meals due to a physical or mental condition.
If you are unable to shop for or prepare your own meals due to a physical or mental condition, and are homebound, you may be eligible to have meals delivered to your home. A small suggested donation is requested for the meal. Eligible participants will be given an opportunity to donate. No eligible participant will be denied a meal because of inability to contribute.

You must have a referral to participate in this program. Referrals for Home-Delivered Meals may be submitted by phone or (for Community Providers and Discharge Planners only) online.
