County Adult Assistance Programs Applying for CAAP

To apply for CAAP, visit our Service Center at 1235 Mission Street during normal business hours (Monday - Friday, 8:00 a.m.-5:00 p.m.), where you will complete a CAAP Screening Form and speak to a staff member who will provide you an application packet and schedule your initial appointment with an Eligibility Worker. 

When you return for your scheduled initial appointment interview, you will discuss your eligibility and learn more about CAAP services and employment opportunities that you may qualify for.

How to Prepare for Your Initial Appointment

You can get a head start by coming to your initial appointment prepared with the following: 

  • Any U.S. state-issued identification card or drivers license 
    Expired identification may be acceptable as well
  • Social Security Number
    You do not need to bring your Social Security Card
  • Verification of your citizenship status 
    Examples include Forms I-551 or I-151 ("Green Card", naturalization certificate, etc.)
  • Verification of residency in San Francisco 
    • If you have a landlord, ask them for a letter with his/her contact information verifying the date you started living there and confirmation that you are paying rent. In addition, we will need verification of the address, such as a utility bill, lease agreement, rent receipt from management, etc. 
    • If you do not have a permanent address at the time of your appointment, you may be able to get a letter of residency from a community agency that you visit regularly for their services.
  • Verification of personal property, if applicable
    Examples include:
    • Bank statements
    • Vehicle registration
    • Life insurance policy documents
    • Retirement account documents
  • Verification of any income
    Examples include:
    • Last pay stubs from your most recent job
    • Most recent stubs from unemployment benefits, disability benefits, SSA retirement benefits, etc.
