IHSS social workers determine home care service needs of older adults and individuals with disabilities who choose to remain safely in their homes.
What Services Are Provided by IHSS?
IHSS pays for a wide variety of home-based services performed by home help workers, including:
Under direction of a licensed health care professional, IHSS can arrange for paramedical services, or provide transportation to and from medical and other necessary appointments.
Who Is Eligible for IHSS?
Residents of San Francisco who are living in their own home (not a board and care facility, nursing home, or hospital), are U.S. citizens, on Medi-Cal, and demonstrate functional needs for assistance with activities of daily living are eligible for IHSS.

Referrals for In-Home Supportive Services may be submitted by:
A client can be referred to IHSS over the phone by calling (415) 355-6700.
Completed IHSS referrals can be faxed to (415) 557-5721.
Completed IHSS referrals can be mailed to Human Services Agency, P.O. Box 7988, San Francisco, CA 94120
ONLY Community Providers and Discharge Planners may submit online referrals at this time. Please note, a username and password must be established at first log-in to the online referral system.
