This employer is a nonprofit supporting young people to move beyond homelessness. Founded in 1984, we’ve helped over 80,000 young adults in San Francisco by providing a continuum of healthcare, housing, employment, and education services. We are San Francisco’s largest nonprofit provider for young people experiencing homelessness. . We serve 1,500 youth annually, operating or partnering to provide more than 500 emergency shelter, transitional, supportive, and subsidy-based housing beds at several unique sites across San Francisco. Case managers will be responsible for providing comprehensive, strength-based case management services to homeless and/or marginally housed transitional aged youth (ages 18-24). Case Managers will conduct a comprehensive evaluation of bio-psychosocial, emotional, educational, vocational and functional needs of young people; develop an individualized service plan in coordination with agency continuum of care as well as leverages relevant community resources as needed. The Case Manager leads collaborative service planning and must have a thorough understanding of the stages of change and development. The Case Manager must be comfortable providing services to clients outside of normal office settings, including but not limited to emergency shelter environments, congregate living housing placements, scattered site housing, and single room occupancy (SRO) placements throughout the city of San Francisco.​​ 

DUTIES:​​ 

  • Conduct intake and assessment; direct service linkage and referral; case planning and contracting; case coordination and advocacy; follow up and after care.​​ 
  • Develops and maintains strong relationships with clients.​​ 
  • Provide crisis prevention, intervention, and safety planning services, along with stress management and life skills development.​​ 
  • Assist young people in applying for and obtaining benefits and housing.​​ 
  • Assist in providing client resources of outside agencies and supportive services that promote the health and wellness of youth.​​ 
  • Work closely with community behavioral health providers and internal service teams to ensure coordinated and effective services.​​ 
  • Maintain accurate detailed observation and progress records and completes necessary electronic data entry into agency and city databases. Identifies trends in data to assist in collaborative service planning.​​ 
  • Adhere to laws regarding confidentiality (HIPAA) and reporting requirements and maintains necessary compliance documentation.​​ 
  • Collaborate with fellow staff members and established community partners, including property managers and landlords.​​ 
  • Facilitate transportation and provides accompaniment to client appointments in the community.​​ 
  • Complete safety and wellness checks regularly in residential units.​​ 
  • Teach life skills (e.g., cooking, cleaning, budgeting).​​ 
  • Participate in organizational meetings​​  
  • Availability to work evenings and weekends when program needs dictate.​​ 
  • Support program services within the milieu. For example, crisis intervention or helping to maintain the safety of the program.​​ 
  • Other general duties contributing towards the care and welfare of the clients we serve.​​ 

MINIMUM QUALIFICATIONS:​​ 

  • One year case management experience mandatory/over one year preferred, preferably with at risk populations from age 18-25 or similar population.​​ 
  • Valid driver's license and clean DMV printout (no recent accidents or moving violations) for operation of company-owned vehicles.​​ 
  • Basic computer skills for data entry, email, outlook calendar management.​​ 
  • Passionate about ending youth homelessness and excitement to serve Transitional Aged Youth (TAY), aged 18 to 25 of diverse backgrounds.​​

  • You work best in an on site, in person working environment. This role is 100% onsite/in person.​​ 
  • You are willing to work holidays occasionally.​​

PREFERRED QUALIFICATIONS:​​ 

  • Over one year case management experience preferred, preferrably with at risk populations from age 18-25 or similar population.​​ 
  • Experience working in drop-in services, shelter, transitional, and/or permanent supportive housing programs.​​ 
  • College degree in public health/social service-related fields.​​ 

SCHEDULE/WORK LOCATIONS:​​ 

Residential Programs located throughout San Francisco, including the Tenderloin and one housing site in Berkeley.​​  

Schedule: Tuesday through Saturday OR Sunday through Thursday.​​  

Hours: 9:00 AM to 5:00 PM OR 10:00 AM to 6:00 PM, depending on site needs.​​ 


Tenderloin Drop-In Center​​ 

Working Days: Monday through Friday.​​ 

Hours: 9:00 AM to 5:00 PM OR 8:00 AM to 4:00 PM, depending on site needs.​​  


*Case managers may be required to visit multiple sites in one day; transportation is reimbursed.​​
