BILINGUAL CASE MANAGER FT

This local social services agency helps homeless families and families at imminent risk for homelessness to achieve housing stability, family well-being, and self-sufficiency. We are the access point for any SF family facing a housing crisis and our services include emergency shelter, transitional housing, rental assistance, early childhood education, mental health support, in addition to a broad spectrum of counseling, parenting education, prevention, and support services. The Bilingual Case Manager works with families who are at risk of or currently experiencing homelessness, providing counseling, support, referrals, and access to resources. The Bilingual Case Manager will meet regularly with clients, both in person and remotely. Depending on which program the case manager is part of, the focus of their work with families will vary. Some programs primarily focus on helping families to secure stable housing; others focus on helping families find employment, find childcare, or create supports to ensure long-term stability and success.
FUNCIONES:
- In partnership with client families, create a set of goals and action plan to move towards long term stability.
- Build supportive relationships with client families and provide services from a trauma informed perspective.
- Provide case management services to anywhere from 10-30 families (exact number varies by program).
- Case management services include counseling, crisis intervention and assessment, as well as referrals and linkages to services, including substance abuse and mental health treatment, childcare, parenting education, support groups, money management, living skills and more.
- Work with clients to secure stable housing and address barriers to housing.
- Maintain up-to-date case files, case notes and family assessments in Salesforce database and other city-wide systems.
- Work in collaboration with other programs and make appropriate referrals to programs as well to external providers and resources.
MINIMUM REQUIREMENTS:
- Fluent Spanish (written and oral) required
- English fluency required (written and oral)
- Valid California Driver's License and clean DMV printout
- At least two years of counseling experience, including crisis intervention
- Relevant work experience in a social services setting
- Understanding of challenges facing homeless and immigrant families
- Understanding of trauma-informed care
- Strong interpersonal and communication skills
- Knowledge of Bay Area housing and social service resources
- Outstanding work habits: punctual, reliable, cooperative, team-oriented
- Demonstrated commitment to diversity, equity, inclusion and belonging work
- Strong computer skills with proficiency in Microsoft Office (Outlook, Word, Excel)
- Must be able to complete online application, including longer form, essay-type questions
PREFERRED QUALIFICATIONS:
- Experience with Salesforce is a plus.
JOB SCHEDULE:
Schedule is 37.5 hours a week. Most case managers work Monday - Friday, though a few roles include a weekend day. Typical hours are 8:30 AM - 5:00 PM or 9:00 AM - 5:30 PM.
