This nonprofit elder-care program was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. Participants receive in-home care services and transportation to a the center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the direction and supervision of the Supervisor/Manager, the Caregiver 2 provides personal care services to participants receiving home care and/or day center services.


DUTIES / RESPONSIBILITIES:

·        Provides personal care/escort assistance according to the individual care plan (i.e. grooming, dressing, peri-care, showers, bathing, toileting, housekeeping, meal preparation/feeding, laundry) and reports observed changes in the participant's condition to the interdisciplinary team.

·        Demonstrates safe practices, by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.

·        Interacts and provides services to participants, families and interdisciplinary team, in a professional manner (i.e. encourage participation in conversation and activities).

·        Provides onboarding and ongoing training for other Caregiver-1s and 2s using manuals and formal training materials as a guide.

·        Active participation in departmental meetings and family meetings as needed.

·        Other related duties as assigned.


QUALIFICATIONS (knowledge, skills, abilities):

1.   Minimum one-year experience working with the frail or elderly required. New CNA graduates OK if they meet this minimum.

2.   Minimum one-year experience working in home or clinic setting preferred.

3.   HHA or CNA required.

4.   Must have current Basic First Aid and CPR certification.

5.   Understanding of PACE model gained through personal experience or research.

6.   Must be flexible and have a positive attitude working across multiple settings.

7.   Ability to work independently and as part of interdisciplinary team.

8.   Knows, uses and explains use of safety equipment for lifting; corrects others in a positive way "here let me help you and show you how this works."

9.   Physical requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands. Must have visual acuity. Must be able to lift/move weight that is within individual physical ability and use appropriate lifting devices or request assistance when needed.

10. Basic English reading and writing required; excellent communication skills regarding interaction with clients and staff.

11. Basic computer skills; able to use or learn to use multiple applications such as email, electronic health record. Some training provided.

12. Have experience in bathing, toileting, and incontinence of care for ALL genders.

 

JOB SCHEDULE:

Monday to Friday – 8am to 4:30pm
