Public Records and Subpoenas

You can make a request for public records by emailing the Human Services Agency's Sunshine Ordinance Coordinator at hsasunshine@sfgov.org

  • In-person at the HSA Investigations office located at 160 S. Van Ness Avenue. 
    Please note that hard copy subpoenas will not be accepted at any other HSA locations. Process servers are not permitted in HSA facilities for the purpose of personally serving an employee in a private matter. Subpoenas of this nature will be accepted at 160 S. Van Ness and forwarded.    

Hearings

Request a State Hearing
Clients of state and federally-funded aid programs, such as: CalWORKs, CalFresh, Medi-Cal, and In-Home Supportive Services, have the option to apply for an administrative fair hearing if they are dissatisfied with an action taken in their case.  Requests for a hearing can be made by calling the California  Department of Social Services at (800) 952-5253.  Written requests for a hearing can be mailed to the Human Services Agency Appeals Unit at S600, P.O. Box 7988, San Francisco, CA 94120-7988.

Request a County Adult Assistance Program (CAAP) Fair Hearing
CAAP clients may request a fair hearing to appeal the withholding, discontinuance or denial of benefits, or the recoupment of overpayments by calling (415) 558-1177 (24 hours).  Written requests can be mailed to: CAAP Fair Hearings #WS20, P.O. Box 7988, San Francisco, CA 94120
