Job Description
Employer is one of the largest community health centers in the United States targeting the medically underserved population. This position assists the residents in performing activities of daily living, consistent with the individualized plan of care. Promotes independence, allowing each resident to maintain the highest level of physical, mental and social well-being possible. Provides a comfortable, clean and safe living environment. Observes residents for changes in their physical or emotional state and promptly reports to the Medication Aide and/or Director of Health Care Services.
Duties:
- Ability to understand and to follow written and verbal directions.
- Able to express self adequately in written and/or oral communication with residents, families, co-workers, and administration.
- Promote and maintain good public relations.
- Provides care in a manner that protects and promotes respect, dignity, and privacy.
- Assist with morning meals for residents as directed
- Demonstrates competency in assisting the resident with all activities of daily living, to include bathing, grooming, dressing, oral care, toileting, transfers and mobility.
- Provides direct care according to plan of care and physician orders.
- Promotes the highest possible quality of life and level of independence for every resident.
- Ongoing observation and reporting of resident status, ability to recognize changes in resident’s conditions as evidenced by physical signs and symptoms, verbal expression or direct observation of changes in their physical, mental, emotional, social or functioning level.
- Demonstrates knowledge of and respect for the rights, dignity and individuality of each resident in all interactions.
- Maintains confidentiality of resident and facility information.
- Acts as a resident advocate when necessary, and is a mandated reporter of abuse.
- Completes required documentation of care and services delivered during shift on the Activities of Daily Living Sheet.
- Utilizes working care plan and 24 hour report form for continuity of care.
- Initiates accident/incident forms, as necessary.
- Reports relevant information to Supervisor including skin condition, compliance issues, resident risk factors, changes of condition, incidents, complaints or dissatisfaction expressed by resident or family, refusals of treatment, loss of resident property and unexplained injuries.
- Attend required orientation, training and on-going in-service education program, and apply all information to job duties. Provide on the job orientation to new employees.
- Working knowledge and ability to comply with safety program, hazard communication, utilize safe work practices, request assistance and/or training as indicated.
- Demonstrates competence in the use of proper body mechanics.
- Utilize infection control procedures, hand washing, standard precautions and personal protective equipment to prevent the spread of infection and promote environmental health.
- Current knowledge of and compliance with all applicable facility policies, procedures and licensing regulations.
- Monitors physical environment for safety and/or fire hazards.
- Knowledge of emergency and disaster plan. Ability to remain calm in case of emergency. Provide first aid, as necessary.
- Sufficient mobility and strength to move freely through the building, to assure resident safety at all times.
- Successfully complete training requirements required by the Department of Social Services, Community Care Licensing Division, Residential Care Facility for the Elderly Regulations and Assisted Living in a timely manner
- Performs other job duties as required by manager/supervisor.
Minimum Qualifications:
- Must be 18 years of age.
- High School Diploma or equivalent preferred.
- Sufficient education to demonstrate functional literacy.
- Previous experience in a health care setting or work with the elderly population preferred.
- Must maintain current First Aid certification
- Must maintain current CPR or BLS certification
- Ability to receive and maintain a California Background Clearance through the Department of Social Services.
- Desire to work with the elderly.
- Good physical and mental health.
- Maintain a well-groomed professional appearance.
- Displays good judgment and common sense.
- Ability to work alone, as well as part of a team.
- Display patience, understanding and compassion.
- Dependable, thorough and accountable.
- Physical ability to spend extended periods of time on their feet.
- Be able to pass the background check and drug test.
- Bilingual in Cantonese or Mandarin.
Schedule: Monday to Friday; 5am-10pm, 8 hrs a day
