Medi-Cal eligibility is based mainly on income. Once you apply, HSA will help determine whether you qualify for Medi-Cal or recommend other affordable healthcare options.
Effective January 1, 2026
No changes to these eligibility rules:
- The monthly income rules for Medi-Cal
- Full coverage for existing Medi-Cal recipients: Californians who already have full coverage should renew their coverage when their Medi-Cal renewal is due to keep their benefits. This applies to all Medi-Cal recipients regardless of immigration status or age.
- Full coverage for children ages 0-18 and pregnant people: They can still enroll in full-scope Medi-Cal, regardless of immigration status. Coverage is for the entire pregnancy and one year after the pregnancy ends. Full-scope Medi-Cal coverage includes doctor visits, dental, vision, and mental health services.
Eligibility changes for undocumented Californians
- If you do not receive Medi-Cal benefits: Undocumented Californians aged 19 and older who currently do not receive Medi-Cal benefits are no longer eligible to enroll for full medical coverage through Medi-Cal. Exceptions apply for undocumented foster youth up to age 26 and pregnant people.
- If you already receive Medi-Cal: You can keep your benefits as long as you renew. If you lost coverage or are uncertain if you qualify for Medi-Cal, contact us online at SFHSA.org/contact-us or call (415) 557-5000
Asset limits reinstated for older adults and people with disabilities
- Asset limits: You will be affected if you are over 65, have a disability, or live in a nursing home. The asset limit for individuals will be $130,000 and $65,000 for each additional household member (up to 10). Important: the home you live in does not count.
- Assets evaluated: When you apply for or renew your Medi-Cal, we will evaluate your assets, such as cash, bank accounts, second vehicles, and second homes.

Don't know if you qualify for Medi-Cal? Even if you don’t, we encourage you to still apply and we can recommend other affordable health care options:
- Covered California: Helps qualified applicants obtain discounted health insurance from well-known companies
- The County Children’s Health Initiative Program (CCHIP): Provides affordable and comprehensive health, dental and vision insurance for children up to age 18 who don't qualify for other publicly funded insurance programs.
- Medi-Cal Access Program (MCAP): Provides healthcare to mid-income, uninsured pregnant women.
Health care for uninsured residents
You can still receive basic, affordable health care services through Healthy San Francisco. This program serves uninsured residents regardless of immigration status, employment status, or pre-existing medical conditions. Learn more about applying at healthysf.org.
