Types of Verification for County Adult Assistance Programs (CAAP)

Types of verification documents
- Birth certificate
- U.S. Passport
- Certificate of Naturalization
- Certificate of birth abroad
- Print-out from the Social Security Administration
- Paycheck stubs received in the month of application
- Letter from employer with net pay and hours worked
- Copy of child support check or payment stub
- Benefits award letter for Social Security, Veterans, Unemployment, Disability, or others
- Self-employment tax forms, such as IRS Schedule C
- Receipts for work expenses if you are self-employed
- School grants/loans/financial aid statements
- Vehicle registration
- Proof of loans, debts, liens on property
- Statement of joint ownership
- Mortgage bill or property deed
- Bank statements
- Life insurance policy, stocks, bonds, IRAs, most recent retirement account statements
- Settlements, such as lawsuits and insurance claims
- School attendance records
- Verification of name change
- Driver’s license or Identification card
- Photo ID from a government agency
- Passport or Certificate of Naturalization
- Documents from the U.S. Citizenship and Immigration Services (USCIS)
- Jail or prison Identification card
- Social Security number (providing card is not necessary)
- Rental agreement or rent receipts
- Mortgage bill
- Utility bill
- Property tax statement
- Hotel or motel receipt
- Statement from a landlord or roommate explaining the housing arrangement
- Eviction notice or notice to pay rent or quit
- Immigration papers, forms, cards
- Form I-551 or I-151 (“Green Card”)
- Order of Supervision from the U.S. Citizenship and Immigration Services (USCIS)
- Other proof of immigration from USCIS, such as work authorization, letter of decision, court order
- Sponsor statement form
- Proof of pregnancy from doctor or clinic, with expected due date
- Doctor statement or disability finding by an agency (SSA/SDI/VA, etc.)
