Disability + Aging Services Healthier Living - Chronic Disease Self-Management
Chronic Disease Self-Management is a free, six-week workshop developed by Stanford to help older adults self-manage their ongoing chronic health conditions. The workshop series focuses on self-management skills, including problem-solving, action planning, healthy eating, behavior modeling, setting and reaching goals.
Workshops are offered by our community partner Onlok and meet once a week for 2.5 hours. Each series is facilitated by two trained health educators and conducted in different languages.
Who is eligible
Any older adult who is living with an ongoing health condition. Caregivers, family, and friends caring for an older adult with an ongoing health condition are also eligible.
