Healthier Living - Chronic Disease Self-Management

Healthier Living is a six-week health education workshop for persons with ongoing health conditions that affect their quality of life.

Healthier Living is a six-week health education workshop for persons with ongoing health conditions that affect their quality of life. Examples include but are not limited to arthritis, stroke, high blood pressure, heart disease, lung disease, chronic pain, Parkinson’s, depression, and HIV.

This evidence-based health education program was developed at the Stanford Patient Education Research Center. Features of the workshop include:

  • No cost to participate, but donations are welcome;
  • Participants attend one 2-1/2 hour class per week for 6-weeks;
  • Classes are offered in different languages at locations throughout the city;
  • Workshops are lead by trained facilitators;
  • Incentives offered for graduates.

Workshop topics include:

  • Understanding chronic conditions and how to manage them;
  • Setting achievable goals;
  • Finding support and solutions to problems;
  • Improving nutrition and eating healthier;
  • Effective exercise and fall prevention;
  • Pain and fatigue management;
  • Relaxation and stress management skills;
  • Medication management
  • Managing difficult emotions;
  • Communicating better with family, friends and health care providers.

Older adults, caregivers, or family & friends caring for someone with an ongoing health condition are ALL welcome.
