Các bước để nhận Medi-Cal​​

   Chuẩn bị nộp đơn​​ 

   Quý vị sẽ cần cung cấp thông tin dưới đây cho tất cả các thành viên trong gia đình. Cơ Quan Dịch Vụ Nhân Sinh San Francisco (San Francisco Human Services Agency, SFHSA) sẽ cố gắng xác minh thông tin của quý vị bằng phương thức điện tử. Nếu không thể xác minh, chúng tôi sẽ yêu cầu quý vị cung cấp tài liệu xác minh.​​ 
    

   • Income information for everyone in your household​​  
   • Tax filing information for anyone in the household who files taxes or is claimed as a dependent (you don’t have to file taxes to qualify for Medi-Cal)​​ 
   • Social Security numbers for applicants who have them or are eligible to have them​​ 
   • Immigration information used to determine your benefit type​​ 
   • Property ownership for those applying on the basis of age (65+) or disability​​

   • In person at one of our Service Centers or community partner locations.​​ 
   • Complete and submit the application (English​​  | Español​​  | 中文​​  | русский​​  | Filipino​​  | Tiếng Việt) bằng một trong những cách sau:​​

     • Fax: (415) 355-2432​​ 
     • Mail:  Human Services Agency, P.O. Box 7988, San Francisco, CA 94120​​ 
     • Service Center drop box: Outside 1440 Harrison Street during business hours, or at 1460 Mission Street at any time.​​

   Sau khi quý vị nộp đơn xin​​ 

   Có thể cần đến 45 ngày để xác định tính đủ điều kiện và lên đến 90 ngày nếu quý vị đăng ký dựa trên tình trạng khuyết tật.  Trong thời gian này:​​ 
    

   • You'll receive a Notice of Action or Request for Information: The notice informs you of your benefits and lists each eligible individual in your household. If we need more information before issuing the notice, you’ll receive the Request for Information.​​ 
     • Disagree with the Notice of Action or need clarification? Contact us at (855) 355-5757. If you still disagree with the decision, you may file for a State Hearing by completing the back of your Notice of Action.​​ 
   • Sau khi đủ điều kiện nhận Medi-Cal, quý vị sẽ nhận được Thẻ Nhận Diện Phúc Lợi (Benefits Identification Card, BIC) mà quý vị có thể sử dụng để bắt đầu nhận bảo hiểm.​​ 
   • Sau đó, quý vị sẽ nhận được một gói hồ sơ qua thư với các chương trình bảo hiểm sức khỏe để lựa chọn. Nếu quý vị không chọn một chương trình trong vòng 30 ngày, tiểu bang sẽ chọn một chương trình cho quý vị.​​
